Patient centered medical home means all healthcare decisions will be made with you, the patient, at the center with your trusted primary care provider and his or her team to guide you. Think of your primary care provider as a general contractor. When building a house, the general contractor keeps all of the other contractors on task and on budget.

In the case of a patient centered medical home, the ‘contractors’ your provider coordinates with for your health include the other providers in their care team, nurse, care coordinators as well as specialists and support services such a radiology, lab and physical therapy.

All of these services work together to support you in managing and organizing your healthcare. Your care team will ensure your care is coordinated among the various services you require with one central base with your primary care provider.

In order for your primary care provider to support your ongoing health, it’s important you receive care from them when you need it. By caring for you when you are sick or injured your primary care provider will have a well-rounded understanding of your healthcare needs long term.

It’s important you communicate with your primary care provider when you have questions or concerns about your health. During office hours, you may call and leave a message with your provider’s nurse; our goal is to return calls within one business day. After hours, you may call MHP’s switchboard at 641.672.3100 and ask for the provider on call. After providing some brief information, you will receive a call back. If your question is not urgent, you are encouraged to utilize the messaging feature of myMHP, the patient portal that provides you access to your personal health information.

Primary care providers partner with you and your care team to ensure you have access to the services you need to make better healthcare decisions.
