Job Description:

Status: Part-time, Shift: 2-4 hours; 3 times a week, 12-24 hours per pay period (every 2 weeks).

Education Requirements: High school graduate or equivalent.  Mandatory Reporter within 6 months of hire.  Basic Life Support. Certification in Activities Coordinator.

Job Requirements: Experience in planning activities for geriatric groups with varying abilities preferred.  Requires Basic Life Support Certification.  Self-motivated, detail-oriented individual able to work with minimum supervision.

Duties: Demonstrates the knowledge and skills necessary to provide service/care appropriate to the age of the patients served, in accordance with MHP standards.  Must have good communication skills and the ability to work well with the geriatric populations.  Responsible fore interviewing each resident upon admission to complete an activity care plan that is individualized with individualized goals based on interview of resident/family/representative.  Responsible for posting activity calendar for each resident on the unit and available for family members if requested.  In some cases must provide 1:1 activities for those residents requiring such need to meet activity goal.  Completes required documentation on residents and maintains other department records in accordance with departmental and unit policies/procedures and state/federal regulatory agencies.  Maintains required department confidentiality and abides by HIPPA regulations.  Observe elders’ attendance, mood, behavior, and involvement so that department activities and resident progress is evaluated.  Reporting, noting, and charting  involvement.
